Medication Therapy Management (MTM)

At Apple Health Solutions, we also have a network of professionals to help suit your staffing needs. We also perform MTMs or Comprehensive Medication Reviews (CMR) at no cost of you.

• We will first setup an account with Mirixa or Outcomes with the pharmacy
• Then we will run reports to determine with patient that would benefit the most from the CMRs
• Our team will coordinate a call or a visit with the patient and will go over all medications at the pharmacy if applicable.
• The PSL will then continue their monitor patient adherence and new medications.
• We will also schedule Miniclinics, which are appointment where we can do diabetic testing training, blood pressure monitoring and assessment patients overall medication adherence and health.
